RT Social Care are working with our long term client, London Borough of Ealing, to recruit for 3 qualified Social workers to joing their Hospital Discharge Team. The role is currently fully remote.

Ealing Council are offering 3 Social Workers, the exciting opportunity to join our hospital team. Ealing is a multicultural borough, and we are looking for social workers who embrace this, within their practise.

We at Ealing have a heart for people. The festive period is fast approaching, and it is essential we support those who we can, to be with their loved ones or in a place of safety to improve their wellbeing. The hospital team prides itself on creating a supportive environment, with a focus on upskilling workers who have some experience in hospital social work. Whilst experience is helpful, we are also looking for social workers who are assiduous.

We are 1 team split into many 3 different areas, and we a privileged to be able to offer training and advice in reablement, assessment and review.

Currently, the hospital team are currently working from home. However, staff are expected to be ready to return when advised. All successful candidates will be required to collect equipment from the office.

The position require a commitment to 35 hours per week. Weekend working availability is mandatory as a when it is required. This will be managed by a rota system.

The nature of the work we do is fast paced and we require a person who discerning. We are looking for a someone who is diligent, ambitious, and vibrant; Who’s experienced in assessment and reviewing care and support; Demonstrates a willingness to learn and can manage complex and challenging adult cases.

Contract Type: Agency
Working Days: 35hrs 7hrs Daily. Weekend Working availability is required.
Location: Currently Working from Home.

Role:
• Social Workers will be to complete section 9 assessments and section 27 reviews, following hospital discharge.
• Proficient in case noting.
• A good knowledge of the care Act and models and Theories pertaining to social work practise.
• Identifying, vulnerable adults and advocate for ongoing care and support.
• Providing handover to maximising their potential to live independently, following assessment.
• Currently the hospital team are prioritising the discharge to assess model. Experience in this sector is welcomed, but not mandatory.
Joining a Hospital Assessment Team means you will be working in a supportive and collaborative environment. The team has a family ethos and enables social workers to develop their practise, whilst improving the wellbeing of those they work with.

What is required:

For your application to be considered you must be able to demonstrate the following:

• you should be a qualified social worker with a current Social Work England registration.
• You’ll bring at least 2 year’s social work experience, with evidence of continuing professional development, and capability of practice in accordance with current professional standards.

Candidates will be required to demonstrate effective communication skills, and an ability to develop a rapport and engage with diverse groups of individuals.
