Qualified Social Worker to join a busy team working as part of the Multi-Disciplinary Team and working collaboratively with health partners facilitating safe and timely discharges from hospital to the patient’s next destination. The IOW “Care Close to Home” strategy is a vision that we have embraced which ensures that “Home” is always the first option for many patients coming out of hospital and preventing unnecessary delays to their journey.
You will be required to work a rota covering Monday – Sunday and the pattern will be on a rota basis.
The ideal candidate will have a good understanding of the role and the complexity of the work undertaken to achieve the best outcomes for people who are being assessed under the Care Act 2014 and the Mental Capacity Act 2005. Ensure a high quality of practice that promotes independence, recovery, choice and autonomy.
• Clear experience and operational knowledge of working in a busy and pressurised front line team
• Have a clear understanding of and experience working with Adult Social Care statutory duties under the Care Act 2014, Mental Health Act, Mental Capacity Act and other primary legislation and guidance including Continuing Health care.
• To demonstrate person centred working
• To facilitate and support individuals to complete a needs assessment using a strengths-based approach, which focuses on the skills and abilities, considers their networks of support both within families and the wider community.
